Tax statements will be available for all members through Online Member Services (OMS) from 10 July 2018.
If you have consented to receive email, you’ll receive a notification that your statement is available in OMS. If you have not provided email consent, we will send your statement via post by 15 July 2018.

|What is the Private Health Insurance Statement?
You’ll need this to complete your tax return. The ATO will use this information to determine whether you’re entitled to the Australian Government Rebate and whether you need to pay the Medicare Levy Surcharge.
The statement lists your share of premium payments that are eligible for the Australian Government Rebate, any rebate you’ve already received, and the number of days you held hospital cover. The premiums and rebate amounts have been divided between all adults who were on your membership at the time the payments were made.
What is the Lifetime Health Cover (LHC) Annual Statement?
The LHC Statement shows any loading applied to your membership, the age you took out hospital cover, and whether there have been any gaps in your Hospital cover. It’s just for your information and – unlike your Private Health Insurance Statement – you don’t need it to complete your tax return.
What is the Standard Information Statement?
It’s a general summary of your cover that we provide to our members once a year. If you’d like more detailed information about your cover, please see our Product Sheets or contact us.
|Who receives the tax statement?
You’ll receive a statement if you were covered by UniHealth as a Member, Spouse, Partner or Defacto at any time during the 2017-18 financial year. Dependent children (even those over the age of 21) won’t receive a statement.
Why have you sent me two statements? One is in the name of my partner/spouse.
All adults who were covered by UniHealth during the financial year receive their own statement, which shows their share of the Australian Government Rebate. The premiums and rebate amounts have been divided between all adults who were on your membership at the time the payments were made.
My ex-partner has been sent a tax statement and received an allocation of the premiums I paid. Why?
We’re required by law to issue a separate tax statement to all adults who were covered by UniHealth during the financial year. The premiums and rebate amounts are divided between all adults who were on your membership at the time the payments were made, even if you paid all the premiums.
Will my child receive a tax statement?
Dependent children on your membership won’t receive a tax statement because they’re not eligible to claim the rebate. They’ll need to copy the policy details from your tax statement if their income is above the threshold for the Medicare Levy Surcharge (above $90,000 for a single person). Refer to the ATO website for more details.
|When will you issue my Private Health Insurance statement?
All statements will be mailed to members by 15 July 2018 and will be available via Online Member Services from 10 July 2018.
The statements include details of all payments processed by UniHealth up to and including 30 June. This means that we have to wait until early July to prepare the statements, but rest assured, we work to get them to you as quickly as possible.
How will I receive my statement?
All members can download their tax statement and LHC statement from Online Member Services (OMS) from 10 July 2018.
If you’ve consented to receive emails from us, you’ll receive an email in early July letting you know when your statement is available on OMS. If you’re on a Couple or Family membership, your partner’s statement will also be on your OMS account.
If you haven’t given us consent to email you, your statement will be posted to you.
|My statement doesn’t show the full amount that I paid for our health insurance last year. Why?
This could be because:
1. There was another adult on your policy at any point during the financial year
The amounts on the statement have been divided between all adults who were on the membership at the time the payment was made.
2. You have a Lifetime Health Cover (LHC) Loading applied to your policy.
The Australian Government Rebate doesn’t apply to LHC loading, and the statement only shows the premiums that are eligible for the Australian Government Rebate.
3. The statement only shows payments received during the 2017-18 financial year
The statement is based on the date payments were received, which may be different to the period of your health cover. Any pre-payments made before 1 July 2017 will show on last year’s tax statement.
Why has my ex-partner been sent a tax statement and received an allocation of the premiums I paid?

How do I tell the total cost of the policy?
If you have a LHC loading, your tax statement won’t show you the total cost of the policy. This is because the statement only shows the premiums that are eligible for the Australian Government Rebate, and the rebate doesn’t apply to LHC loading.
If you don’t have a LHC loading on your policy, you can calculate your total payments by subtracting the amounts in Your Australian Government Rebate received from the amounts in Your premiums eligible for Australian Government Rebate (i.e. subtract label K from label J).
Remember, each person’s tax statement lists their share of the premiums, so if you had a spouse or partner on your membership at any time during the 2017/18 financial year, you’ll also need to include the figures on their tax statement in your calculations.
Why doesn’t the statement show any payments for my health insurance this year?
The tax statements only display payments received by UniHealth during the period 1 July 2017 to 30 June 2018. If you have pre-paid your contributions in the previous financial year, they won’t show up in this year’s statement.
|What is the Australian Government Rebate?
The Australian Government Rebate is an amount that the government pays towards your health insurance premiums. It’s income tested, so if you have a higher income you may receive less rebate. Everyone who is eligible for Medicare and whose taxable income falls into the relevant bracket can claim the Australian Government Rebate.
How do I tell what tier I’m on?
You can check which tier is currently applied to your membership by logging into Online Member Services and going to the Payments -- Contribution Details page. You can change your rebate tier by editing your Contribution Details.

How do I calculate which tier I should nominate?

The tiers are based on your income for Medicare levy surcharge purposes. For more information and to use an income calculator contact the Australian Taxation Office (ATO) on 13 28 61 or visit ato.gov.au. If your tier has changed for the 2017-18 financial year, don’t forget to let us know.
What happens if I nominate the wrong tier?
There’s no penalty for nominating the wrong tier. If you receive the incorrect amount of rebate throughout the year, you’ll either receive a tax offset or a tax liability through your tax return at the end of the financial year.

|Can I claim the Net Medical Expense Tax Offset (NMETO)?
The NMETO allows you to claim a deduction on your out-of-pocket eligible medical expenses if they are over a certain amount in the previous financial year. To find out if you’re eligible to claim the offset, visit the ATO website.
How do I get a Tax Claims statement for the NMETO?
To download a copy of your Tax Claims statement, log into Online Member Services and click on the Benefit Statements link in the Quick Links section on the home page.
